Thymosin Alpha-1 (often written Tα1) is a natural with 28 building blocks. The brand name is Zadaxin.
Its job is to tune up your immune system. It helps your T-cells (immune fighters) mature and signal better.
It is approved in more than 35 countries for chronic hepatitis B and as a vaccine helper. It is not in the US. Americans mostly get it through compounding pharmacies.
It has a strong safety record over about 30 years of use.
What people use it for.
Abroad, it is mainly used for hepatitis B and C, and to help vaccines work better.
In the US, people use it off-label for immune support, long COVID, chronic fatigue, and sometimes alongside cancer care.
It does not have proof for "immune support" in healthy people. That use is not validated.
Does it actually work?
For its approved uses, like hepatitis, yes. The evidence supports it.
For long COVID and chronic fatigue, the proof is thin. The most-cited human paper (Liu 2020) had only 76 people and was observational, not a randomized trial. A big sepsis trial (ETESIS, 405 people) missed its main goal, though it hinted at help in some subgroups.
So: well-proven for some specific diseases, mostly unproven for the trendy uses people buy it for.

It is well tolerated. You may get mild redness where you inject.
The serious stuff
The big one is the direction it pushes your immune system. Tα1 turns the immune dial up. That is the wrong direction if your immune system is already overactive.
- Do not use it if you have an autoimmune disease like rheumatoid arthritis, lupus, , IBD, or an active Hashimoto's flare. It can make autoimmune problems worse.
- Do not use it on transplant immune-suppressing drugs without your transplant team.

Where people get it.
From safest to riskiest.
- Brand (Zadaxin). Not available in the US. It is approved abroad but cannot be legally bought by US patients.
- . The main US path, by prescription, about $180-$400 a month. Its legal status is contested, and some pharmacies have stopped selling it.
- Research-only (). Sold "for research only," about $80-$200 a month. Highest risk. A mass spec identity check is the only honest test.
- . Overseas raw. The main vendor list shows it suspended, so supply is patchy.

Our honest take.
Under a specialist, for hepatitis or alongside cancer care, the evidence is real and this earns its place. That is the narrow lane it belongs in.
For a healthy adult chasing general "immune support," it is a different story. Turning the immune dial up sounds great until you remember which way that points if your immune system is already overactive, and the autoimmune-flare risk is the part the ads leave out.
So the guardrail is simple: any autoimmune condition, even a quiet one, takes this off the table for you.
