Semax

A Russian stroke-and-focus peptide sold under one name in three different versions, and the one you will probably get is not the version with any trial behind it.

In plain words.

Semax is a small brain . It is made of seven amino acids.

It comes from a piece of a stress hormone called ACTH. Scientists removed the hormone part and kept the brain part. It was made by the same Russian lab that made Selank.

In Russia it is an approved medicine, going back to 1997, stocked in regular hospitals the way an everyday prescription drug is here. Doctors there use it for stroke recovery and for ADHD in kids.

Outside Russia, it is not a medicine. It is sold "for research only." So you are looking at a drug with a real track record in one country and almost none anywhere else.

What people use it for.

In Russia, doctors use it for recovery after a stroke or a head injury, and for ADHD in children.

Outside Russia, people mostly try it for focus and mental clarity. It is popular with tech workers, finance folks, and students who want something that is not another stimulant.

Honest note: a healthy adult using it for "focus" is far outside what the real trials studied. The trials were about brain injury, not sharpening a healthy brain.

Does it actually work?

Maybe, after a brain injury, but every bit of that comes from Russian trials nobody outside has been able to vet.

There are zero English-language randomized trials. The Russian studies looked at stroke and at ADHD in kids. There are about 60 animal studies. But like Selank, it is hard to check how those Russian trials were run from outside.

There is also a name trap. Three different molecules get sold as "Semax": plain Semax, N-acetyl Semax, and N-acetyl Semax amidate. The Russian trial proof is for plain Semax only. Most sellers actually ship the amidate version, which was not the one in the trials.

A DIY mix sprayed up your nose is a fast route to your blood, and a dirty mix is a real risk. It also nudges serotonin and dopamine, so pairing it with stimulants or antidepressants stacks an untested risk.

How you take it.

You take it in the nose, as a spray. That is the route the Russian studies used.

Watch the dose gap. Russian stroke trials used 1,000 mcg a day or more. The common online dose is only about 300 mcg a day, roughly one-third of the trial amount. So the gym dose is much smaller than the studied dose, and that gap is not written on any bottle.

Do not inject it. The data is for the nose route. If you mix powder into a nose spray yourself, you must keep it clean. Your nose is a fast route to your blood, so a dirty mix is a real risk, not a small detail.

What to watch out for.

The serious stuff

Keep it away from drugs that push brain chemicals: antidepressants (SSRIs, SNRIs, MAOIs), tramadol, triptans, lithium, and stimulants. The Russian reports say Semax nudges both serotonin and dopamine, so pairing it with a stimulant or an antidepressant stacks an untested risk on top of an untested drug.

If you DIY a nose spray from powder, contamination is a real worry. The sealed Russian product is sterile. Your kitchen mix is not. Use clean, careful technique.

The common stuff

The Russian reports did not flag major side effects. But that is one country's papers, not a proven global safety record.

Where people get it.

Here are the lanes from safest to riskiest.

  • Brand (Russia). A real approved product, about $30 to $60 a month at Russian retail. But bringing it into the US is legally restricted.
  • . A few US pharmacies make it, about $90 to $220 a month, usually as a nose spray. This is the closest US-legal match.
  • Research-only (). The main lane outside Russia, about $25 to $70 a month. Sold "for research only." You usually mix your own nose spray.
  • . Overseas raw powder, about $7 to $9 a month. Unregulated import.

Read the label closely. Make sure you know which of the three versions you are buying, because only plain Semax has the trial evidence.

Our honest take.

Three molecules get sold as "Semax," and the proof only covers one of them
Russian doctors have used the plain version after strokes for decades. The amidate version most sellers actually ship was never in those trials. Real history, shaky proof, a coin flip on what is in the vial.

If a neurologist is steering your recovery from a stroke or head injury, a short, careful nose-spray run has a real frame around it. As a daily focus pill for a healthy brain, you would be dosing a fraction of the studied amount on a use the trials never looked at. The powder route only makes sense if you confirm which of the three versions you actually have, since only plain Semax carries the evidence, and it stays in your nose, never a needle.
