- you've heard "Ozempic" everywhere and want to know if it's right for you
- your cardiologist mentioned the SELECT MACE result and you have established heart disease
- you're a type 2 diabetic and your A1c is creeping despite metformin
- you're shopping a compounded semaglutide vial from a cash-pay tele-clinic — the FDA shortage closed in **May 2025**, so the legal cover for new 503A compounding is gone. NovoCare self-pay at **$199–$349/mo** is now usually cheaper than the compounded route, and it's the real molecule
- you or a first-degree relative have medullary thyroid carcinoma (MTC) or MEN-2 — this is the FDA black-box stop. Self-sourcing skips the prescriber gate that's supposed to catch it
- you've had pancreatitis — Wegovy PI Section 5.2, discontinue if suspected
- you take levothyroxine, warfarin, oral diabetes meds, or oral contraceptives — slowed gastric emptying changes absorption; Wegovy PI Section 7.1 says monitor more frequently
- you're not prepared for what stopping looks like — STEP-1 extension (Wilding 2022, Diabetes Obes Metab): participants regained **about two-thirds of lost weight within 1 year off-drug**. Plan to take it **indefinitely**, or plan for that regain
- Pancreatitis-like pain (severe upper-abdominal, won't sit still): stop and call your doctor.
- Gallbladder pain or persistent vomiting that won't resolve.
- Severe muscle loss without resistance training (the 'GLP-1 face' is real — protein floor matters).
- 12 weeks at full dose with zero weight movement (rare, but real).
What it is.
Semaglutide is a synthetic that mimics GLP-1, a gut hormone your body releases after eating. The molecule is engineered to resist enzymatic breakdown, which is why one injection lasts a week.
It's sold by Novo Nordisk as Ozempic for type 2 diabetes and Wegovy for chronic weight management. Same active ingredient, different label and dose ramp. There's also an oral form (Rybelsus), which is less popular because the bioavailability math is unkind.
Semaglutide is what people usually mean when they say "." It was the first member of this class to land cultural escape velocity, and it remains the reference compound everything else in the category gets compared to.
