Read this if
  • you have a teenager 12+ with obesity and Saxenda is on the table — it's the only on-label GLP-1 for that age
  • you're a parent of a 12–17-year-old whose pediatric endocrinologist mentioned liraglutide — it's the only on-label GLP-1 for that age group (Saxenda, ages 12+ for obesity). Daily injection, supervised under endocrinology, not a shortcut around teen-specific lifestyle work
  • you're cost-sensitive and the new generic is finally affordable
  • you couldn't tolerate semaglutide's GI profile and want to step down to a shorter half-life
Skip this if
  • you can tolerate a weekly injection — the weeklies hit harder and hurt your routine less

First 90 days · Liraglutide
Weeks 1–2
GI side effects start. Nausea is the rate-limiter. Don't titrate up if you can't tolerate the starting dose — the side-effect curve gets steeper, not flatter.
Weeks 4–8
Appetite suppression becomes the obvious effect. 1–2 lbs/week is typical. Hit a plateau? Hold the dose, don't immediately bump.
Week 12 — decide
Bloodwork: A1c, lipids, liver enzymes, fasting insulin. Visible weight change should be 5–10%. If the trajectory is flat at 12 weeks and you've titrated, this isn't your molecule.
Quit if
  • Pancreatitis-like pain (severe upper-abdominal, won't sit still): stop and call your doctor.
  • Gallbladder pain or persistent vomiting that won't resolve.
  • Severe muscle loss without resistance training (the 'GLP-1 face' is real — protein floor matters).
  • 12 weeks at full dose with zero weight movement (rare, but real).
Identity

What it is.

Liraglutide is the original modern GLP-1 agonist. Novo Nordisk approved it for type 2 diabetes in 2010 (as Victoza) and for weight management in 2014 (as Saxenda). Same molecule, different label.

The molecular engineering is older — a single fatty-acid chain attached to the native sequence to slow enzymatic breakdown. It works for about 24 hours, which is why it's daily, not weekly.

It's been mostly displaced by semaglutide and tirzepatide for new starts, but it still has a real role: it's now off-patent, generic versions are arriving, and it's the only with FDA approval down to age 12 for pediatric obesity.
