In plain words.

Gonadorelin is a lab-made copy of GnRH. GnRH stands for gonadotropin-releasing hormone. It is the signal your brain sends to your pituitary gland.

That signal tells the pituitary to make two more hormones, LH and FSH. Those then tell your testicles to make testosterone and sperm. So gonadorelin sits at the very top of the chain.

Your natural GnRH comes out in little bursts, not a steady stream. The science word for that is . This timing detail turns out to be the whole story with this drug.

It has been for decades, but only for a diagnostic test and a special fertility pump.

What people use it for.

The big modern use is in TRT clinics. TRT means testosterone replacement therapy.

When men take testosterone, their own signal shuts off, and their testicles can shrink. Clinics give gonadorelin to keep the testicles active. It is used as an alternative to a drug called hCG.

It is also used, with a special pump, to start fertility in men whose brain signal is broken. And it is used in a quick test to check how the pituitary responds.

Does it actually work?

Yes, but only if you dose it the right way. The timing is everything.

Here is the key fact. The same drug does opposite things depending on how often you take it.

  • In little pulses, every 60 to 120 minutes, it turns the system ON. This raises LH and FSH. With a pump, it brought back sperm in about 80% of men with a certain brain-signal problem.
  • As a steady drip, or very frequent dosing, it does the opposite. It shuts the system DOWN. This is the same trick a cancer drug called Lupron uses.
  • Dosed just twice a week, as some clinics do, it mostly does nothing reliable. The drug only lasts 2 to 4 minutes in your body, so the signal is long gone before the next dose.

So if a clinic dosed you twice a week for fertility, that is not the timing that works.

The drug only lasts 2 to 4 minutes in your body, so a twice-weekly shot is long gone before the next dose. Timing matters more than total dose here, and that schedule is below the threshold that does anything reliable.

How you take it.

You take it as a shot under the skin. How often depends on the goal.

  • For keeping testicles active on TRT, clinics often use 100–200 mcg daily, or twice daily. Honest note: even daily may be too weak to truly protect fertility.
  • For starting fertility, a special pump gives tiny pulses of 5–25 mcg every 90 to 120 minutes. That is the proven setup.
  • The twice-a-week plan some clinics use is below the threshold that does anything useful. It is more of a convenience than a real protocol.

Frequency matters more than the total dose here.

What to watch out for.

The common stuff

Serious side effects are uncommon at the doses TRT clinics use. It is generally well tolerated.

The serious stuff

  • The biggest danger is using it wrong. Continuous or too-frequent dosing shuts your hormone system down instead of supporting it.
  • Do not use it if you have a pituitary tumor or another problem in this hormone system that has not been checked out by a doctor.

Three questions to ask your clinic: What dose interval are you using? What blood labs are you tracking (LH, FSH, testosterone, estradiol)? And when do we stop if it is not working?

If they are not drawing those labs, they cannot tell if it is doing anything. Talk to a real doctor. We are not your doctor.

Where people get it.

Here are the ways people get it, safest to riskiest.

  • Brand (Factrel). , but only for a diagnostic test, not daily TRT use. About $200+ per vial.
  • . This is the main TRT channel. Made by a pharmacy with a prescription, about $80–$150 a month. It became the go-to choice after hCG got hard to find in 2022.
  • Research-only (). Exists, about $40–$70 a month. But is so easy to get that few people bother.
  • . Barely any market here, since is cheap and easy.

For this one, the clinic route is both the safest and one of the cheapest. There is little reason to chase the .

Our honest take.

It can work, but the timing is the whole game
If your clinic doses it daily and tracks your labs, it can reasonably keep your testicles active on TRT. If they dose it twice a week, that is too weak to count on.

For most TRT users, we would actually ask the clinic about hCG instead. It has more evidence for this same job.

If you want fertility back, you belong in a real fertility clinic on a pump or an hCG plan, not on a casual twice-weekly shot. And if your clinic only offers gonadorelin, ask them to walk you through their reasoning and which labs they will check.
