A pooled re-analysis of the two terminated MK-677 elderly-frailty phase 2b trials confirms a meaningful congestive heart failure signal: 6.5% incidence at 25mg/d vs 1.7% on placebo over 12 months (p=0.014).
Why it matters:
the off-label community has been using MK-677 at 12.5–25mg/d for years on the assumption that the original CHF signal was "just elderly frailty noise." The re-analysis stratifies by age and baseline cardiac function — the signal persists in healthy adults under 65.
Mechanism is plausible: MK-677 raises GH/ and causes fluid retention. Combined with cortisol-stimulating effect and increased aldosterone, the cardiac load thesis holds.
